Definition

Mild neurocognitive disorder is defined as modest cognitive decline from a previous level of performance in one or more cognitive domains, based on concern from the individual, a knowledgeable informant, or the clinician, along with objective evidence of impairment, where the deficits do not interfere with the capacity for independence in everyday activities, though greater effort, compensatory strategies or accommodation may be required.

Causes

MCI can be an early stage of an underlying neurodegenerative process, most often Alzheimer's disease pathology, in which case it may progress to dementia over subsequent years. It can also result from vascular risk factors, depression, chronic sleep deprivation, certain medications, thyroid dysfunction, vitamin B12 deficiency, or excessive alcohol use — causes that are often at least partly modifiable or reversible. Distinguishing between a likely neurodegenerative and a likely reversible or vascular cause, through history, examination and targeted investigation, shapes both the monitoring plan and realistic expectations for the person and family.

Brain Mechanisms

In vascular-related MCI, small-vessel disease disrupts white-matter connections important for processing speed and executive function. Cognitive reserve — built through education, ongoing mental stimulation, and cardiovascular health — is thought to explain why people with similar amounts of underlying brain change can show quite different levels of clinical impairment, and why lifestyle measures remain relevant even after a diagnosis of MCI.

Diagnostic Criteria (Patient-Friendly)

  • Evidence of modest cognitive decline from a previous level in one or more cognitive domains, based on concern from the individual, an informant, or the clinician
  • Objective evidence of modest impairment on cognitive testing, typically in a range below expectation for age and education but above the threshold for major impairment
  • The cognitive deficits do not interfere with capacity for independence in everyday activities, though more effort or compensatory strategies may be needed
  • The deficits do not occur exclusively in the context of delirium
  • The deficits are not better explained by another mental disorder, such as depression

Differential Diagnosis

Conditions a psychiatrist will consider and rule out before confirming this diagnosis:

Major neurocognitive disorder (dementia)
The key distinguishing feature is preserved independence in daily activities in MCI, versus significant functional impairment in dementia.
Normal age-related cognitive change
Occasional, non-progressive forgetfulness without objective decline on testing and without informant concern is more consistent with normal ageing than MCI.
Depression
Depression in older adults commonly presents with concentration and memory complaints and can be difficult to distinguish from early cognitive decline without careful assessment and, sometimes, a treatment trial.
Delirium and medication effects
Both should be actively excluded, since they can produce a similar clinical picture that resolves once the underlying cause is treated.

Investigations

  • Formal cognitive testing (such as MoCA), ideally with a baseline for future comparison
  • Assessment of mood, sleep and medication use
  • Blood tests: thyroid function, vitamin B12, folate, and metabolic screening
  • Brain imaging where the presentation, age of onset or examination findings suggest a specific structural or vascular cause
  • Functional assessment confirming preserved independence in daily activities

Treatment

There is no medication currently approved specifically to treat MCI itself, so management centres on identifying and treating any contributing or reversible causes, addressing cardiovascular and lifestyle risk factors that influence progression risk, and establishing a monitoring plan with periodic reassessment, typically every six to twelve months or sooner if symptoms change. This is also an opportunity to have proactive conversations with the person and family about future planning while cognition and decision-making capacity remain largely intact, which is far preferable to having those conversations later under pressure.

Prognosis

Outcomes in MCI vary: a proportion of people progress to dementia over the following years, particularly when an underlying neurodegenerative process is present, but a meaningful proportion remain stable, and some improve, especially when a treatable contributor such as depression, sleep apnoea or a vitamin deficiency is identified and treated. Regular follow-up allows progression to be identified early if it occurs, and gives the best opportunity to address modifiable risk factors that may slow decline.

Encouraging regular follow-up, supporting a heart-healthy and socially active lifestyle, and having early, unpressured conversations about future wishes and planning are some of the most useful things a family can do at this stage.

Does MCI always progress to dementia?

No. While MCI does increase the risk of later dementia compared with the general population, a meaningful proportion of people with MCI remain stable over time, and some improve, particularly if a treatable contributing cause is found and addressed.

Is there a test that can predict who will progress to dementia?

MCI itself is not an emergency, but seek prompt medical assessment for any sudden or rapidly progressive change in cognition or alertness, which can indicate delirium or another acute medical problem rather than typical MCI, and seek psychiatric assessment if low mood, hopelessness or thoughts of self-harm accompany the cognitive symptoms.
